Medicare is: a health benefit program for US citizens and people living in the US legally as permanent residents that are:

  • Age 65 or older
  • Under age 65 with certain disabilities
  • People with End Stage Renal Disease

Part A: Hospital Coverage (original Medicare)

  • Inpatient hospital care
  • Skilled nursing facility care
  • Some home health care
  • Hospice care

Part B: Medical coverage (original Medicare)

  • Health care providers (Doctors)
  • Outpatient care
  • Durable medical equipment
  • Preventive services (limited)

Part C: Medicare Health Plans

Part D: Prescription Drug Coverage

Medigap Plans (Supplemental plans) are not considered a part of Medicare but are helpful if you choose to stay with original Medicare.

When you turn 65:
If you want Part A and Part B you will need to contact Social Security

(you can apply online at http://www.socialsecurity.gov/medicare/apply.html ) and request the coverage.

You can start this process 3 months prior to your birthday. If you plan to remain on your group health plan you might want to delay Medicare (check with your group admin at work to see how your plan works with Medicare).

If you are already receiving Social Security benefits you will automatically be signed up for Parts A and B effective the first day of the month you turn 65.

Cost:

  • Part A: premium free if you or your spouse paid Medicare taxes while working for 10 years
  • Part B: premium cost is $135.50 in 2019 (if you have a high income you could pay more)
  • Part C: depends on the plan you purchase
  • Part D: depends on the plan you purchase (if you have a high income you could pay more)

You can stick with original Medicare (consisting of Parts A and B) and add Part D drug coverage. Then you have the option to add a Medigap plan (supplemental coverage), and other coverage options like dental and vision.

Medigap Plans (Supplemental plans)

  • Only available to purchase with original Medicare Parts A and B. You will also need to purchase your Part D drug coverage separately (if you choose to do so).
  • Limited guaranteed issue period of 6 months.
  • Not available if you buy Part C medicare plan.

You can join a Part C plan which is a Medicare Plan

  • You will still have Medicare but you will get your Part A and Part B coverage from your Medicare Plan, not original Medicare, offered by private companies approved by Medicare
  • Required to cover all of the services that original Medicare covers
  • Cost Sharing equal to original Medicare
  • Required to have an Out of Pocket Maximum
  • Many offer Part D drug coverage. These are called MAPD plans.
  • Usually offers extra coverage beyond what original Medicare offers
  • There are different types of Medicare Plans
